Texas Health & Safety Code §181.105 (House Bill 4224
Effective September 1, 2025, Texas Health & Safety Code §181.105 (House Bill 4224, 89th Regular Session) requires Texas-licensed health care providers to make the following information publicly available: how to request your health care records, how to contact the provider's licensing authority, and how to file a consumer complaint. How to request your health care records
You have the right to request copies of your health care records. To do so, contact us using one of the following methods:

Please include your full name, date of birth, preferred contact information, the dates of service requested, and how you would like your records delivered.
Additional authorization or identity verification may be required before records are released. Requests are processed in accordance with applicable state and federal law, including HIPAA and Texas Health & Safety Code Chapter 611. Reasonable, cost-based copy fees may apply when permitted by law.
